How Do I Request My Medical Records from Virtua?
Virtua Health offers several ways to obtain your medical records.
1. MyChart - Your Personal Health Information Online
The easiest and most convenient way to access your medical records (for records from October 2017 to present) is via MyChart, a free, secure online tool that enables you to access your medical information at home or on the go and communicate with your health care team. Visit MyChart for more information on how to sign up or log in.
Please note that if you need access to your medical records from prior to October 2017, MyChart will direct you back to this webpage to request your medical records via one of the methods below.
2. Other ways to request your medical records:
For Hospital Records: Mail or fax your specific request (providing a date or an approximate date of your appointment, procedure, or hospital stay), along with a completed Release of Patient Information (Spanish Version) form to the appropriate Health Information Management Department (Medical Records) from which you need your records.

For Virtua Medical Group (VMG) Records If you are a patient of a VMG practice and want to receive a paper copy of your records, complete the VMG Release of Information Form and forward it directly to the VMG practice. For mailing and fax options, click to locate information for the specific practice you seek.
3. In-person requests
Present your request, authorization, and government-issued photo identification to the Health Information Management Department (Medical Records) at one of the above-listed addresses.
Hours of operation are Monday through Friday, between 8:30 am and 5 pm (excluding holidays).
