Below you’ll find answers to common questions about the MyChart patient portal. View all FAQs.
To sign up for MyChart, you must be 18 years old. (For patients aged 12-17, the process for signing up is different. Patients in this age group should talk to their doctor at their next appointment about creating a MyChart account.) You also need to provide an email address and you’ll be provided a one-time activation code.
You can get this activation code using one of these ways:
- Request one online. You’ll need to enter some personal information so we can verify your identity. This includes your legal name, address, date of birth, and social security number.
- Ask for one. You can request an activation code when you check in for an appointment at your Virtua Medical Group provider’s office. You can also ask a member of your care team if you are an inpatient at the hospital.
- Review your After Visit Summary. You’ll find a MyChart activation code on your After Visit Summary that you receive after any Virtua patient visit.
- Submit a form. Download and complete our request form. Then fax it to 856-762-2843, or scan and email it to MyChartPHR@virtua.org.
As soon as you have your activation code, you can enroll in MyChart.
When you log in for the first time, you’ll be prompted to create a username and password. From that point forward, you’ll use that username and password whenever you want to sign in to MyChart.
If you’re the parent or legal guardian of a child under 18, there are two ways to request proxy access:
- Through your own MyChart account. Log in, then select Menu. Scroll down to Sharing and select Minor Proxy Access Request Form. Once you’ve completed the form, click on Submit Request.
